Service Agreement and Policies
Cancellation/Rescheduling
- We request that you provide 24+ hours of notice if you need to change or cancel an appointment. Failure to do so prohibits us from serving other pets in need and may result in a rescheduling fee equal to the cost of the visit fee.
- EUTHANASIA EXCEPTION: To cancel or reschedule a euthanasia appointment please provide a minimum of 2 hours advanced notice. Failure to do so will result in a rescheduling fee of $50.
Payment
- Payment is due at the time services are rendered. Payment plans or delayed billing are not available.
- Acceptable forms of payment include: cash, checks and credit cards (Visa, Mastercard, Discover & American Express). There is a $25 fee for returned checks
- Care Credit is NOT an accepted form of payment
Service
- All requests for appointments, consultation, or medical care must be submitted via website, e-mail OR phone call . Text messaging will be used for appointment confirmation and arrival times only
- GYVS does not have the capabilities to perform x-rays, surgery, or hospitalize patients for intensive care. GYVS is NOT available on an emergency basis.
- If you are unsure if your pet is experiencing a medical emergency it is advised to contact your local emergency veterinary clinic.
Tolerance
- GYVS strives for a positive environment for our clients and staff. We require staff and clients to treat each other with respect and kindness. We are in this together for the safety and well being of your beloved pet(s).
- There is a 2-strike policy regarding ill treatment of our staff. The second warning is the last and we will mail your pets medical records to you.
